Apply for a library card and get instant access to select online collections and services*!

Complete the form below to receive a temporary library card number. Convert your temporary card** to a permanent card by visiting Centerville Library or Woodbourne Library and presenting the following:

  • A valid, non-expired photo ID, issued by a U.S. federal or state agency, with current address. (e.g., driver’s license, passport, or military ID) OR
  • If photo ID does not have current address, present photo ID and separate proof of current Ohio address (e.g., recent utility bill, pay stub, piece of mail with name and current address)

NOTE: A parent or guardian must accompany minors under age 18 to verify address and convert a temporary library card to a permanent card.


Teacher cards and Homebound cards are not eligible for online application. Please visit or call the Library for application process information.


*These online collections CANNOT be accessed with a temporary card: Freegal, Hoopla and Kanopy.

**Temporary cards, not converted to permanent cards, will be deleted after 6 months.
